Yes, you can import MBOX files into Apple Mail with attachments intact if the MBOX file is properly structured and free of corruption. The manual import option works well for small and healthy MBOX files. By selecting Import Mailboxes from the File menu and choosing the MBOX format, emails are generally imported along with their original attachments, formatting, and folder hierarchy.

However, I have faced issues when dealing with large or slightly corrupted MBOX files. In some cases, attachments were not displayed correctly, or certain emails were partially imported.
